Supported Employment Services Provider Requirements
Participating students must be working for pay and receive supported employment services at an integrated work setting.
The provider of supported employment services must:
- Be a nonprofit corporation under s. 501(c)(3) of the Internal Revenue Code
- Serve Hardee County, DeSoto County, Manatee County, or Sarasota County
- Contract with an eligible private school
In order to participate in the pilot program, an eligible Supported Employment Services Provider must submit, by fax or mail, the form below:

In order to participate in the pilot program, an eligible Supported Employment Services Provider must receive from a participating private school, and submit to the Office of Independent Education and Parental Choice, by fax or mail, copies of the forms below:
