Colleges must have an up to date Authorized Signature form on file. If the college wishes to designate additional authorized signers for certifying data submissions, they must submit a new Authorized Signature form.

The Authorized Signature form is available for download for colleges to submit authorized signers for database certifications as well as report the appointment of new Reports Coordinators.

The following fillable PDF forms are available for certifying submitted data.

All forms should be completed, printed, signed, and scanned as PDF files. Specific instructions for naming convention, completing and submitting these forms via TIBCO are located on each form.
